OFFICIAL PRE-REGISTRATION FORM

February 10, 2011 COMPETITION

· Plan to arrive at Leu Gardens in time for the 8:00 a.m. registration.

· This form must be completed and received by mail or fax to the following office before December 17, 2010: Osceola Soil and Water Conservation District, Attn: Heidi Lockwood, 1921 Kissimmee Valley Lane, Kissimmee, FL 34744, Fax to(407) 847-0959, Attn: Heidi Lockwood.

· All team members and their advisors/teachers must wear their Envirothon t-shirts at Leu Gardens the day of the event.

· Participants are not allowed to take backpacks, resource materials or books into Leu Gardens .

· Team alternates will not attend the Regional Envirothon unless replacing an absent team member.

· Each team should bring a clipboard and a pencil.

· Schools may enter no more than six (6) teams.

· Once registered, teams may not change their names.
